Candidates can obtain the application form for all the courses offered by the college through online and offline mode. Candidates can obtain the application form from the institute office. Applicants can apply for the State and National level exams through the respective official websites.

Following is the procedure for applying online for the courses are:

• Download the Application Form in the PDF format

Candidates must make a DD of INR 1100/- for A-Category and INR 2600/- for B-Category towards registration and application form fee from any bank in favor of “Shadan Institute of Medical Sciences”.

• MBBS attempt certificate (for PG courses)
• Class 10 Marksheet and Passing Certificate
• Class 12 Marksheet and Passing Certificate
• NOC and Employer’s certificate (in case of employed)
• One-year of Rotating Internship Certificate (applicable for PG Courses)
• Copy of Passport/ Driving License/ PAN Card/ Voter ID as a proof of residence of Parents.
• Graduation Certificates (for PG courses)
• Caste Certificate (applicable in case of SC/ ST/ OBC)
• Certificate in support of having three years of service for seeking admissions to MD (community health)
• Disability Certificate (if applicable)
