• Candidates are required to obtain the prospectus and application form either in personal from the campus or via speed post
• Application form must be filled in candidate’s own handwriting
• Fill out all the details including personal information and academics in the space provided
• Attach recent photograph in the space provided
• Enclose all the relevant documents along with original fee receipt
• Submit the duly filled application form at the campus either in person or by speed post before the due date

• Original fee receipt
• Attested photocopy of mark sheets and certificates of examinations passed since HSC
• Attested photocopy of CLC and conduct certificate from last attended institution
• One attested passport size photograph to be pasted at the space provided
• Caste certificate and income certificate, if applicable
• Ex-servicemen certificate, PH certificate, or any other certificate claiming weightage or reservation in admission, if applicable
• Duly filled acknowledgement card
• One Self attested unstamped envelope of size 28cmX12cm
