The application form of LVPEI is available on the official website. Interested candidates need to follow below-listed steps in order to apply for the desired course.

1. Go to the official website of LVPEI i.e www.lvpei.org
2. Click on the Application Form link.
3. Enter all the details required in the application.
4. Also upload documents such as photographs, 10th certificate, 12th certificate, etc.
5. Post filling the form, proceed to pay application fee through a demand draft in the name of “Hyderabad Eye Institute” payable at Hyderabad.
6. Lastly, submit the duly filled application form.
